What the inspector wrote

On 11/5/25 at 8:30am, Licensing Program Analyst (LPA), Anna Chan conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Assistant Director Gelizly Vargas. Facility is requesting to be licensed from 28 to 42 school-age capacity and to utilize Room # 10 and add Room #9 to their license. Hours of operation are Monday through Friday 6:30am to 6:30pm. No children were present during today\342\200\231s inspection. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Gelizly Vargas that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and used previously measured indoor and outdoor activity space on 12/07/22. Total indoor activity space measured was 2211 sqft, which is sufficient to accommodate the requested capacity of 42 Children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via Brita water pitcher . LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/03/2025. LPA observed a total of 4 sinks and 4 toilets available for children\342\200\231s use. However, 1 toilet in room # 9 is out of order. LPA tested the toilet in Room #9, the water rises close to the rim and flushes water down very slowly. Assistant Director stated they will put a work order to fix the flush. However, with 3 toilets, it is still sufficient to accommodate the requested capacity of 42 children. Page 1 of 2 The facility currently has a fully fenced playground area. Chain link fencing is at least four feet high. The total square footage for the outdoor activity space is 6336 sqft, which is sufficient to accommodate the requested capacity. Shade is provided via Sunport shade, a tent looking shade that is anchored on the ground. LPA observed there were no outdoor age-appropriate toys and play equipment available on the playground. LPA observed a play structure without manufacturer\342\200\231s sticker, Assistant Director stated they will request for a sticker that will show the age. Assistant Director also stated they ordered new toys and equipment that is age appropriate as well. There is adequate cushioning in fall zones of climber provided by rubberized cushioning. Drinking water is available via Brita water pitcher. LPA did not observe any hazardous items on the playground. Assistant Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. LPA verified Lead Testing was conducted on 7/19/24 without Lead Exceedance. The following corrections are needed prior to the issuance of the license: Age-appropriate toys and equipment. Age-appropriate play structure with manufacturers sticker/manual. Licensee Representative Gelizly Vargas understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Exit interview conducted and report was reviewed with Assistant Director Gelizly Vargas . Appeal Rights were provided. A notice of site visit was given to Assistant Director Gelizly Vargas and must remain posted on for 30 days. Page 2 of 2
