What the inspector wrote

Page 9099C Complainant alleged that a child got hit, pushed, body slammed or even slapped in the face and none of these incidents were documented. During the staff interviews, all 5 interviewed staff gave consistent disclosures regarding the facility's discipline policy, including talking to the child, notifying the director, and informing parents as needed. All five out of five interviewed staff stated they do their best to always keep the children safe. We were unable to determine who C1 is. LPA Nguyen attempted to interview the children in care but only 5 were qualify for the interview. The five children interviewed stated teachers are very nice, and they like coming to school. No disclosures were made by children. LPA Nguyen called 10 parents, LPA was able to interview 5 parents on 06/19/23. All parents interviewed made no disclosures, parents stated that the facility communicates with parents on what is going on with their child(ren) when needed. Complainant also alleged that the facility does not report incidents to parents. All 5 staff disclosed facility procedure to address an injury is to let the director know, then the director notifies parents via phone or at pick up time, depending on seriousness of injury. LPA Nguyen called 10 parents, LPA was able to interview 5 parents on 06/19/23. All parents interviewed made no disclosures, parents stated that the facility communicates with parents on what is going on with their child(ren) when needed. During record review, the Parent Handbook, stated the injuries policy, which include parent will be called immediately if facility observed a head injury or in the event of any serious or questionable injury. LPA reviewed Unusual Incident Reports (UIRs) for the month of May and no incidents, with regards to children hitting each-other. Continued on 9099 Page 2 Page 9099C Page 2 Based on the information gathered from LPA's interviews with complainant, 5 staff members, 5 preschool children, 5 parents, and reviewing records of facility's Parent Handbook, there is insufficient evidence to corroborate the allegations that staff did not prevent day care child from being hit by other day care children and staff did not report incidents to day care child’s authorized representative. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED. Exit interview was conducted with Director Deborah, Warkentien. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
