Follow-up on corrections·August 26, 2025

On 8/26/2025, at approximately 9:08AM, Licensing Program Analysts (LPAs), S. Jung and K. Trinh conducted an unannounced Case Management - Deficiency inspection. Upon arrival, LPAs met with Assistant Director, Ashley Ibanez, and were led on a tour of the facility. Observed during the tour were 67 children and 14 staff members present. During today's inspection, the facility was operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates 1 out of 14 facility staff or other individuals who require caregiver background checks did not receive criminal record and child abuse index clearances or exemptions; see LIC 809D for deficiency cited. Staff 1 (S1) who did not receive criminal record and child abuse index clearance was relieved from their shift immediately to bring the facility back into compliance. S1 also did not have a complete personnel record on file; see LIC 809D for deficiency cited. LPAs S. Jung and K. Trinh informed Assistant Director, Ashley Ibanez, that this report dated 8/26/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPAs S. Jung and K. Trinh informed Assistant Director, Ashley Ibanez, to provide a copy of this licensing report dated 8/26/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
