What the inspector wrote

On 9/17/2025, Licensing Program Analyst (LPA), K. Trinh and Licensing Program Manager (LPM), T. Nguyen conducted an unannounced case management-other inspection. Upon arrival, LPA met with Director, Alycia Gardiner. LPA disclosed the purpose of the inspection and was led on a tour of the facility. There were 50 children and 8 staff members present. Hours of operation are 6:30AM to 6:30PM, Monday through Friday. The facility informed LPAs S. Jung and K. Trinh on 8/15/25 during an inspection that they are in the process of removing the raised garden beds located on the preschool playgrounds. LPAs observed that the raised garden beds had been dug out, their walls lined with a protective liner, and an accumulation of dirt/rocks on the rear end of the preschool playground. Director stated that everything will be removed over the weekend (8/16/25-8/17/25), and that the children are not currently using the playgrounds. During today’s inspection, LPA and LPM observed that the gardening bed walls were still under construction. The facility removed the accumulation of dirt and rocks on the rear end of the preschool playground. Director stated that they have been in communication with their corporate office and are still waiting on directives; see LIC 9102 for Technical Violation. Exit interview conducted and report was reviewed with the Director, Alycia Gardiner. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100. (Continue to page 2) (Page 2) Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
