What the inspector wrote

On 3/27/2026, Licensing Program Analyst (LPA) K. Trinh conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 3/26/2026. LPA met with Assistant Director Ashley Ibanez. LPA observed 26 preschool children with 4 preschool staff. Director, Alycia Gardiner arrived at approximately 9:00AM. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 3/26/2026 a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 3/26/2026 at approximately 2:15PM, Assistant Director (AD), Ashley Ibanez, heard Child 1 (C1) crying during nap time in the Preschool B classroom. AD brought C1 to the office to calm them, and C1 shared that Staff 1 (S1) pulled their arm and that they hit their leg on the side of the cot. AD checked C1's leg and did not observe any visible injuries. S1 stated they held C1 by the wrist and guided them to their cot because they were sitting on a chair. The Director contacted the facility's District Leader and was instructed to place S1 on administrative leave pending further investigation. The Director also notified the company risk department via email. During today’s visit, LPA interviewed two staff and obtained pertinent documents. Due to insufficient information available at this time, the reported incident needs further investigation. Exit interview was conducted. The Notice of Site Visit was posted. Director Alycia Gardina was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. End of Report.
