What the inspector wrote

Page 1 of 3 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing the capacity by adding two preschool rooms to the existing Infant/Toddler Program. LPA met with director, Alycia Tucker who gave LPA a tour of the facility. Currently the facility is licensed for 24 infants and toddles ages 6 weeks to 24 months in 3 rooms. LPA observed a total of six staff with 14 children in these rooms. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility has requested to provide care and supervision to 48 infants and toddlers ages birth to 24 months old in 5 rooms (Infant # 1, Infant # 2, Toddler # 1, Toddler # 2, and Toddler # 3 , Monday to Friday from 6:30 am to 6:30 pm and one Friday a month until 10:00 pm. Children are dropped off, signed in and out electronically at the entrance of the building leading to rooms. The facility has met all the posting requirements. All the classrooms were measured. The indoor activity space measurement for the current requested capacity is as follows: Infant Room #1 is: 362.33 square feet. (After deducting 180.51 square feet for napping area) Infant Room # 2 is: 242.99 square feet. (After deducting 104.00 square feet for napping area) Toddler Room # 1 is: 404.63 square feet Toddler Room # 2 is: 421.96 square feet Toddler Room # 3 is: 421.68 square feet Total indoor activity space is: 1853.59 square feet divided by 35 square feet =52.96 ( 53 children). Total toilets: 4 x 15 = 60 children Total sinks: 11 X 15 = 165 children Continued on page 2 Page 2 of 3 Sinks are located in each classroom. All the classrooms have diaper changing tables with attached sinks and raised sides at least 3 inches. The director explained that mirrors are installed above the diaper changing tables for better supervision of other children while a staff is changing diapers since the changing tables are facing the wall of the room. LPA discussed the procedures of diaper changing with director. The procedure for diaper changing were posted above each changing table. The director was reminded that children shall not be left unattended on the changing table. Total outdoor activity space measurement is as follows: Outdoor space for the infant/toddler playground is: 4401 square feet divided by 75 = 58.68 ( 59 children). There are enough cushioning materials under the play structure. No hazardous items observed on the playground. The facility is equipped with carbon monoxide detector. All the classrooms have age appropriate toys and equipment. A copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided to the applicant/facility representative. Continued on page 3 Page 3 of 3 The safe sleep regulations and logging every 15 minutes check on infants and toddlers under 24 months old was reviewed. A sample of the log was given. LPA also discussed the items not allowed in the childcare facilities for infants and toddlers. A flyer of the banned items was given to director. Incidental Medical Services – IMS. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Alycia Tucker. Based on today’s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for increasing the capacity for 48 infants and toddlers ages birth to 24 months old in five rooms will be final reviewed and discussed with management in the Regional Office. The facility will be notified if any additional information is required. End of report.
