What the inspector wrote

On 8/26/2025 at approximately 9:08AM, Licensing Program Analysts (LPAs) S. Jung and K. Trinh conducted an unannounced Case Management inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPAs met with Assistant Director, Ashley Ibanez. LPAs disclosed the purpose of the inspection and were led on a tour of the facility. There were 26 children and eight (8) staff members present. Hours of operation are Monday through Friday 6:30AM to 6:30PM. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. An Unusual Incident Report (UIR) was submitted to the Department on 8/15/2025 reporting suspected abuse toward Child 1 (C1) by Staff 1 (S1). Parent 1 (P1) was picking up their child on 8/14/2025 and allegedly observed S1 choking C1. P1 notified Director of what was observed. Director and Staff 2 (S2) immediately went to the classroom to observe C1. Director and S2 observed C1 to be happy and free of red marks or signs of choking on their body. Interviews were conducted with staff members present on the day of the alleged incident. Staff interviewed did not provide any information to support P1's allegation. P1 could not be reached for interview. Facility staff reported the incident to the Department in a timely manner. Based on the information obtained there were no violations found. No deficiencies cited and report was reviewed with Director, Alycia Gardener. Notice of Site Visit was given and must remain posted for 30 days.
