What the inspector wrote

On 9/17/2025 Licensing Program Analyst (LPA) K. Trinh and Licensing Program Manager (LPM) T. Nguyen conducted an unannounced case management inspection in regards to a Confirmation of Removal. The letter stated that this individual must be removed because their criminal record exemption had been denied. There was a total of 28 children 8 staff present. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Based on the instruction letter from the Department of Justice and communication with the Regional Office, the director confirmed that the individual was removed June 27, 2025. The following were discussed: Individuals who are 18 years of age or older living or working in the home must be fingerprinted cleared prior to being present in the facility. Adults must contact a LiveScan office complete LIC9163. If adult is fingerprinted cleared and associated to another facility, licensee must complete a Criminal Record Clearances or Exemption Transfer Request form. In the areas that were evaluated, the facility was in compliance of the California Code of Regulations, Title 22, Division 12. The licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100.
