What the inspector wrote

Licensing Program Analyst (LPA), Tina Nguyen conducted an onsite inspection for the purpose of a 1- Year Required. LPA and facility director Veronica Gonzalez toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. The overall census observed was 5 staff members and 16 infant children. Facility hours are 6:30 a.m. - 6:30 p.m ., Monday to Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/d ay up to $500.00 maximum per day/per person will be assessed if this regulation is violated. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Care and supervision was evaluated to determine if the basic needs of infants are met and appropriate. Infant Needs and Assessments are done quarterly or updated as needed. Sign-in and out sheets and procedures were reviewed. Personal Rights of infants were observed by LPA. Staff and Infant Records were reviewed for completeness. Inspection of required forms made. Director stated there is a always a staff in the crib room when there is napping infant(s). Changing table has raised sides at least three inches high and is within an arm\342\200\231s reach of a hand washing sink. Toys observed to be clean. Indoor classroom was inspected to ensure that the floors have a surface that is safe and clean. Infants bedding materials are provided by school , they are changed daily or as needed. if needed Center does provide an additional linen. All the linens are washed on premises. Storage for children's belongings was reviewed. Food preparation areas were toured for safety, cleanliness and proper equipment. Meals are provided by parents only. Continued on Page 2 Page 2 Bottles, dishes, and containers were labeled with infants\342\200\231 names were visible on all children's bottles and other refrigerated items. A current menu was posted. Food prep areas appear clean and sanitary. The facility takes measures to keep the facility free of flies, other insects, and rodents. Food is properly stored. Bottles and food containers brought from home are properly labeled with name and date. Trash receptacles containing solid waste have tight fitting lids. Firearms and other weapons are not allowed or stored on the premises. Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. The playground is completely fenced and fencing is in good repair. The playground equipment appeared in safe condition, and the play area is free from hazards. There is sufficient cushioning (soft Rubber cushioning underneath climbing structures and play equipment to absorb falls. Sign in/out procedure was reviewed for compliance. LPA Nguyen advised Facility Director that the infants need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Infants use their own sippy-cups with their name written on the cups when outdoors. Play area was inspected for hazards and inaccessibility to bodies of water. No hazards observed. Menus were posted where they could be reviewed by parents. Solid waste receptacles have a tight-fitting cover and are in good repair. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. The facility has conducted an emergency drill within the past six months. The facility has a working carbon monoxide detector and fire extinguisher. The California Child Passenger Safety Law was posted by the entrance of the facility. Four staff files were reviewed. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a childcare if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for staff were reviewed and within compliance. At least one staff member present possesses current CPR/First Aid certifications, which expires 09/07/2023. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. Continued on Page 3 Page 3 Children's records were reviewed, and there was a separate, complete and current record for each child. A random sample of 5 children's files were reviewed for documentation of the child\342\200\231s name, address, and telephone number of the child\342\200\231s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached when necessary (LIC 700) and a medical assessment. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. The director was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative. Exit interview conducted and report was reviewed with facility director. Appeal Rights were explained. The facility representative was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the Regional Manager to the address listed above. The Notice of Site Visit was posted and discussed as required by H&S Code Sec. 1596.817. Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00. The Notice of Site Visit must be posted on or adjacent to the door.
