What the inspector wrote

On 11/17/2025, LPA Navar conducted a case management inspection and met with Program Lead Maribel Ortega. Upon arrival there were 3 staff present and 0 children in care. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. A self-reported incident by the facility was received at the regional office on 11/14/25 that stated S1 was taking medication stored in a lockbox for children who no longer are enrolled at the facility to drop it off at the pharmacy and that their vehicle was broken into and the lock box was stolen. During this investigation, LPA conducted 1 (one) staff interview (S1) and was informed no other staff knew of the incident, obtained a copy of the children roster, reviewed storage of medication and IMS plans for 4 (four) children in care. During staff interview, S1 stated that they were cleaning the shed located outdoors and that they had found a locked box with 2 (two) medications in it for 2 (two) children who no longer attend the program. LPA observed the shed between Portable classroom 1 and Portable classroom 2 to be inaccessible to children in care by a key lock. S1 stated that C1 has been dis-enrolled since 12/2023 and C2 has been dis-enrolled since 11/2024. During records review for 4 (four) children in care LPA found facility to be in compliance with Title 22 Regulations 101226 Health-Related Services. LPA reviewed Title 22 Regulations 101226(e)(6) Health-Related Services and provided a copy to Program Lead Maribel Ortega. Based on the information gathered from the interview conducted, observation and records reviews, it was determined that no deficiency will be given at this time. Exit interview conducted and report was reviewed with Program Lead Maribel Ortega. A notice of site visit was given and must remain posted for 30 days.
