Page 2 of 3 On 8/4/21 LPA interviewed the director and 3 other staff regarding the allegation. According to director, it was brought up to her attention by the child's representative that child # 1 had some brises on the child's arm one day in June 2021. Child's representative was not sure how the child got those bruises. Director acknowledged seeing the bruises on the child's arm when showed to her. She said she saw 3 small bruises underneath the right arm of child # 1. Director stated child had the tendency to throw oneself on the floor. Staff were careful that child did not throw oneself on the ground outside. Children here loved child # 1. The director stated It might be possible somebody prevented child of falling ended up bruises on the child's arm, but not intentionally. Child might have thrown oneself on the tricycle ride. The director stated she had observed that. The three other staff who were interviewed on 8/5/21 stated the child was loving and they did not witness any bruises on the child's arm. Today LPA reviewed internal documentation of Child Supervision Record (CSR) for the last week on the child's attendance in June. LPA made sure to interview the main teachers of child # 1 (Staff # 1, Staff # 2, and Staff # 3). According to CSR, LPA observed there were other staff in and out in the classroom with child # 1 indoor and outdoor for a short time every day. LPA interviewed those staff as well (Total of eight staff). They said they did not remember witnessing child falling down or sustain bruises. Child # 1 did not have any ouch report issued. Some staff stated child needed 3 staff to change the child's diaper due to the child being defiant. Based on the interviews conducted with the staff who were involved with the child the last week of the child's attendance (Total of 12 staff) and reviewing child's file, there is not enough proof or documents to support if the allegation happened at this facility. This agency has investigated the complaint alleging "child sustained bruises on the arm while in daycare "; although the allegation may have happened or is valid, there is not a preponderance of evidence to prove, the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted. In the area which was investigated, no deficiency was cited today. This report ends here.
