What the inspector wrote

Page 1 of 1 On 9/7/21 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management of the preschool for an incident which was reported to our office on 9/7/21. The Covid-19 Emergency Response questionnaire was reviewed and answered by the director, Mojgan Vasseghi before entering the facility. LPA toured the facility and took census. There were a total of 39 preschool children with 5 staff observed. A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. According to the facility self report, it states a 4 year old child disclosed to her parent that child hugged and kissed a male teacher. Child also disclosed to her parent that another 5 year old child touched the child's private part the week before. Today LPA discussed the incident report with the director and obtained additional information. LPA interviewed the staff mentioned on the incident report. The staff stated staff was covering lunch break in the classroom for another staff. Child # 1 made an art and craft. Child gave it to the teacher saying I made this for you. Chid gave a side hug to the staff. LPA interviewed the child. Child denied anybody hugging or kissing the child. The name on the incident report did not match the name child mentioned during interview. LPA also interviewed child # 2 mentioning on the incident report. Child # 2 denied being friend with child # 1. Child # 2 stated she has never played with child # 1 at this facility. Based on the information gathered there is not enough evidence for occurring any violation at this time. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted. In the area which was investigated, no deficiency was cited today.
