Inspector’s narrative

What the inspector wrote

On 5/9/2025 at approximately 8:55AM, Licensing Program Analysts (LPAs), Christine Jung and Kathy Trinh, conducted an unannounced case management inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPAs met with Director, Mojgan Vasseghi. LPAs disclosed the purpose of the inspection and was led on a tour of the facility. There were no children and staff members present during the time of inspection. Hours of operation are Monday through Friday 6:30AM to 6:30PM. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. An unusual incident report (UIR) was submitted to the Department on 02/21/2025 reporting an incident between staff and children. The facility reported that on 02/19/2025, Child 1 (C1) hit another child and teacher while in care. The facility also reported that C1’s hair was allegedly pulled while in care. The facility staff reported the incident to the Department in a timely manner. During investigation, LPA conducted interviews with staff members and day care children and also conducted file reviews. All individuals interviewed stated that they did not observe C1 hitting another child. All interviewees did not provide evidence to support that C1's hair was pulled at any time. Based on the information obtained there were no violations found. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Mojgan Vasseghi.
