What the inspector wrote

On 8/19/2025 at 9:30am, Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an Annual/Random visit. LPA met with Lead Teacher Jenena Martinez. LPA toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Census was taken. There were 20 preschool children with 5 staff. Children were having indoor activities when LPA arrived. Program Specialist Linda Prieto arrived to continue with the visit. During the inspection, it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 8:00a.m.- 3:00p.m., Monday through Friday. The facility is located inside Commonwealth Elementary School Campus. The preschool program offers AM and PM classes (AM 8:00am-11:00am and PM 12:00pm-3:00pm) A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Facility provides breakfast for AM class and lunch for PM class. Drinking water is available to children indoors by water fountain, children bringing their own water bottles from home with the child\342\200\231s name on it. The children's bathrooms are clean and sanitary. Page 1 of 3 Children do not nap in the facility. The facility has a working carbon monoxide detector and fire extinguisher. The facility conducted an emergency drill last 8/15/2025. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility. The outdoor activity space was inspected for compliance. The playground is completely fenced. The surface of the outdoor activity space was well maintained and free of hazards. The playground uses black top surfacing around the climbing structure which appears to be enough to absorb falls. The outdoor equipment and toys were in good repair and free of sharp edges. Drinking water is available to children outdoors by water fountain, children bringing their own water bottles from home with the child\342\200\231s name on it. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. 4 Staff files were reviewed for staff present during the facility inspection on this date. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Based on records reviewed 2 of 4 staff has expired TB test which is more than 1 year from employment at the facility. 1 of 4 staff hired in 2022 did not have mandated reporter training certificate. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 10/2026. 5 Children's records were reviewed, and there was a separate, complete and current record for each child. Sign in/out procedure through Learning Genie was reviewed. A QR code is used for sign in and out, each child\342\200\231s representative has their own Login ID. In the areas reviewed the children\342\200\231s files were found to be in full compliance. Currently there are no medications in the facility. The facility representative was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative. Based on LPA\342\200\231s record reviews and interviews the following two (2) Type B violation was observed are being cited in accordance with Health and Safety Code sections 1596.8662(b)(1) Administration of Child Day Care Licensing and 101216(g)(1) Personnel Requirements. S ee violation on the attached LIC 809D. An Inspection and exit interview were completed with Program Specialist Linda Prieto. The report was reviewed and discussed.
