What the inspector wrote

(Page 2 of Report) During the course of investigation, LPA Tran and LPA Castanon inspected the facility outside and inside and interviewed 4 staff. During the inspection, LPA Tran and LPA Castanon observed vermin's droppings on the playground area and in the classroom Blue Bird, Red Robin, and in the facility kitchen, along the walls and behind furniture. Director immediately cleaned up the droppings throughout the inspection. LPAs also observed facility have mouse's trap by the entrance of the playground and by the front entrance of the facility. During interviews, LPA Tran interviewed 4 staff at the facility on 10/11/2022. 3 out of 4 interviewed staff said they have seen vermin in the outdoor areas at approximately 2-6 months ago. Based on the information gathered from LPA's observations and interviews conducted with 4 staff on 10/11/2022, the preponderance of evidence standard has been met, therefore the above allegation of facility has vermin is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 1, Section 101238(a)(1) Buildings and Grounds is being cited on attached LIC 809 D. Appeal Rights and deficiency were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Susana Mendoza. (End of Report)
