What the inspector wrote

Page 2 Complaining party (CP) reported on 9/19/22 while bathing Child #1 (C1) they observed a red mark on C1’s bottom from a diaper rash and C1 had multiple diaper rashes while in care. CP stated on one occasion during pick up C1 was crying and saying the word hungry. CP provided pictures. During the investigation LPA Odom interviewed licensee, director, 2 staff members, conducted a facility inspection, and reviewed the children’s roster, personnel report, staff personnel records, child personnel record, diapering and toileting policies, daily schedule, snack menu, and daily reports on ProCare App. LPA did not interview children due to children’s age, unable to qualify for interviews. During the investigation, Director (S1) was interviewed on 01/04/23. S1 stated staff have a daily schedule posted in the classroom and provided to parents of the times children are given bathroom breaks and diaper change which are every 2 hours. If children are being potty trained 1 staff will take the potty-training children to the restroom and the other staff will change diapers for the children in diapers. If a child has a diaper rash, they will inform parents to sign the consent form to apply ointment and every diaper change is posted on the ProCare App where parents can check daily throughout the day. S1 stated they do not recall C1 having diaper rashes while in care, but they do recall C1 having a wet diaper often C1’s diaper was changed 5-6 times per day. Staff are trained on diaper change and potty training. S1 stated sometime in October they transitioned from catered meals to all families providing their meals for their child. Due to not enough parents signing up for catered meals they decided to cancel catered meals and only provide AM and PM snack. S1 stated they cannot force the child to eat their food, but they are constantly encouraging the child to eat their meals. Staff will communicate with parents if the child did not eat their meal and make suggestions of different food the child saw their friends eating. If a parent forgets to pack a meal, they have frozen meals available on site. During the investigation, Licensee (S4) was interviewed on 01/10/23. S4 stated while S1 was on vacation, on 9/30/22 S4 spoke to CP regarding diaper rash, and they came up with a plan to make sure C1 does not get diaper rash. On 9/30/22 S4 did not observe C1 with a diaper rash or a red mark on C1’s bottom. CP did not address any concerns with multiple diaper rashes while in care. S4 disclosed there wasn’t any issues between Staff #5 (S5) and parents. S5 provided good care to the childcare children. Continue to page 3 Page 3 During the investigation, LPA attempted to interview 3 staff members however only 2 were available for interviews on 01/04/22. Both staff disclosed that they check children’s diapers every two hours, and they don’t have any children with diaper rash. If they observe a child with a diaper rash, they will inform parents and document it on the ProCare App. They also document diaper change, nap time, meals, and daily activities. Both staff disclosed if a child is still hungry during snack time, they will give the child more and if a child is still hungry after they ate their lunch, they will inform their parents to pack a bigger meal, but it’s rare when that happens because parents pack a good amount of food. LPA Odom attempted to interview 10 parents however only 3 parents were available for interviews on 1/10/23 and 1/20/23. None of the parents disclosed any concerns, all the parents are satisfied with the childcare center. All the parents disclosed that the childcare center does meet their child’s needs. During facility inspection LPA Odom observed the snack menu was posted at the facility entrance and classrooms. Staff were washing children’s hands and serving a good portion of golf fish to each child. None of the children’s diapers looked full or heavy. LPA observed children were taken for restroom breaks a minimum of 2 times in the AM. Based on LPA’s facility inspection, observations, interviews conducted with licensee, director, 2 staff, 3 parents and records reviewed it was determined there was insufficient evidence that facility staff are not changing children’s diapers in a timely manner and children not having enough food. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. Exit interview was conducted with Director Susana Mendoza. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
