What the inspector wrote
On 1/14/2025, Licensing Program Analyst (LPA), K. Navar conducted an on site inspection for the purpose of a Annual Visit. LPA met with Director Susana Mendoza. LPA and Director toured the facility inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 5 infants and 2 staff.
During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.- 6:00p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials such as books, puzzles, blocks are provided. Food is prepared on site; AM/PM snacks are provided by the facility and lunch is brought from home. Bottles and food containers are labeled with infant names. Food prep areas were clean and sanitary food is properly stored. Menus were posted where they could be reviewed by parents in the office and classroom. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Water cups are refilled as needed by staff.
Infants nap on cots/cribs, and bedding is stored individually, and blankets and sheets are brought from home. Bedding is taken home on infants last day child is care and brought back on infants first day back in care. The facility has conducted an emergency drill on 1/07/2024. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility.

The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material around playground equipment is rubber mats. Infants bring water cups labeled with their names and staff will refill as needed. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair.
Staff files were reviewed for staff present during the facility inspection on this date, 3 staff files were reviewed. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a center iif he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance.
Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete Child Mandated Reporter training, and to renew the training every two years. 3 staffs Child Mandated Reporter is found to be in compliance. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 08/25.
5 Children's records were reviewed, and there was a separate, complete, and current record for each child. LIC9227 or Individual Infant Sleeping Plan and 15-minute Infant safe sleep logs were reviewed and found to be in compliance. Sign in/out procedure was reviewed for compliance. Facility uses ProCare to document sleep checks, diaper changes, feedings, and singing in/out. Person responsible for signing child in and out is given an individual pin that documents time and is linked to parents full legal signature/name.
LPA spoke with director regarding medication. Director stated there are no infants in care who have medication at this time. LPA reviewed procedure for storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018,
requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
The facility has done Lead Test on December 15, 2022.
Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
An Inspection and exit interview were completed with Director Susana Mendoza. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed.
The facility representative was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. Page 3 of 4
