What the inspector wrote

On 02/19/2026, an unannounced case management inspection was conducted by Licensing Program Analyst (LPA) V. Trinh in response to a self reported incident dated 02/12/2026. LPA met with Sarah Pham (Program Director),and Victoria Hernandez (Site Supervisor), and was led on a tour. There were 15 toddler children and 4 staff members, 36 preschool children and 6 staff present at the time of inspection. Victoria Hernandez (Site Supervisor) stated that the total enrollment is 112, and that children are on a staggered schedule and arrive at different times throughout the day, and are aware that they cannot exceed the capacity ratio of 112 children at any one time. During the inspection, it was determined that the facility is operating within its licensed capacity and in compliance with staff-to-child ratios. Hours of operation are 7:00 AM-6:00 PM, Monday through Friday. A review of the Facility Personnel Report Summary conducted on this date, 02/19/2026, indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Department received an incident report on 02/12/2026 stating that on 02/11/2026, Child 1 (C1) sustained an injury to his left arm while playing in the playhouse. Staff approached and provided assistance to C1 by applying an ice pack and administering first aid. HR then shared the video footage with Program Director, Sarah Pham, and Site Supervisor, Victoria Hernandez. During the investigation, LPA conducted interviews with two (2) staff members and one (1) child. LPA obtained pertinent documents for investigative review. Due to additional information needed, the reported incident needs further investigation. Continued Page 2. page 2. Exit interview conducted, and the report was reviewed with Facility Representative Victoria Hernandez. A notice of site visit was given and must remain posted for 30 days. The Director was provided a copy of their appeal rights, and their signature on this form acknowledges receipt of these rights. End of report.
