What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite case management inspection for the purpose of investigating an incident that the facility self reported to our office on 11/6/2025. LPA met with director, Barbara Brown. LPA took census. There were a total of 11 preschool children with 2 staff in one classroom. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. According to the self reported incident, on 11/6//2025, Staff # 1 (S1) left S2 with 14 preschool children to go on 10 minutes break, leaving S2 out of ratio for 10 minutes. Today LPA interviewed the staff who were involved in the above reported incident. (S1, S2, and S3). Staff were interviewed. S1 admitted that S1 did not count the children when they left for 10 minutes break. S1 did not realize the number of preschool children were 14 when they left. S4 had informed S1 that she was permitted to go on their ten minutes break. S1 stated they made a mistake not counting the children when they left. During interview with S2, they said they had 14 children outside on the playground on 11/6/2025 when S2 left for ten minutes break. S2 did not realize this before S1 was leaving. S2 stated they realized they had 14 children after five minutes passed the break time of S1. S2 called the management and Continued on page 2 Page 2 of 2 management stayed with S2 until S2 returned from their 10 minutes break. S3 stated they reviewed the process of checking name to face of children and paying attention to the number recorded on the Child Supervision Record (CSR) which is an internal form the facility uses with S1. LPA also reviewed the CSR for the date of 11/6/2025 and learned that S2 went on their 10 minutes break from 3:27 to 3:37 pm. S1, and S2 admitted there were 14 preschool children outside on the playground. Based on the self reported incident, and interviews 3 staff today, and reviewing Child Supervision Record (CSR) dated 11/6/2025, it was determined that the facility was in violation of teacher child ratio leaving S2 with 14 preschool children on the playground between 5 to 10 minutes. The following deficiency was cited of the California Code of Regulations, Title 22, Division 12 on today's investigation regarding the incident of teacher child ratio. Please see LIC 809D on next page for details of the deficiency. Barbara Brown, the director was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. The director, Barbara Brown, was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with director, Barbara Brown. End of report.
