What the inspector wrote

Page 1 of 4 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite inspection for the purpose of Annual inspection. LPA met with director, Savannah Garcia. LPA toured of the facility inside and outside. The floor and yard plan (LIC 999) were checked. LPA took census, there were a total of 15 preschool children with 2 staff in two different rooms. Census was as below: 9 preschool children with one staff in one room, and 6 preschool children with one staff in another room. Two other rooms were not in use. All the four classrooms were inspected. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30 a.m.- 6:30 p.m., Monday through Friday. Parents sign their children in and out electronically and manually if need to. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. The facility provides AM, PM snacks and lunch to children. Menu is posted. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Drinking water is available to children indoors by water containers that children bring from home Continue to page 2 Page 2 of 4 and the facility fills them up with filtered water if need to. Preschool children nap on cots, and beddings are stored individually and washed on a weekly basis by the facility. The facility has been conducted an emergency drill within the past six months. The last fire drill was conducted on 2/25/2026. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement . The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material on the preschool playground is appeared to be enough to absorb falls.(commercially built foam) under climbing structure observed. Drinking water in the outdoor activity space is provided by water containers children brought from home and the facility fills them up with filtered water. The outdoor equipment and toys were in good repair and free of sharp edges. Shade is provided by canopies. There are no bodies of water present at the facility. Staff files were reviewed for staff present during the facility inspection on this date, 5 staff files were reviewed. They were in compliance. Children's records were reviewed, and there was a separate, complete and current record for each child. A random sample of 5 children's files were reviewed for documentation of the child’s name, address, and they were in compliance. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA also informed director, the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and Continued on page 3 Page 3 of 4 recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code Section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing completed accordance to the Written Directives outlined in PIN 21-21.1-CCP . LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) The facility completed testing on 1/5/2023 and no lead exceedance. . Incidental Medical Services (IMS) discussed. For information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. This requirement is not met as evidenced by: Based on a complaint report, interviewing staff, admission of S1, S2, S3 and reviewing Child Supervision Record (CSR) that S1 was left alone with 13 preschool childrenfor 10 minutes on 4/21/2026 until S2 arrived. This is a potential risk to the health and safety of children in care. An immediate civil penalty of 250.00 is hereby assessed due to a repeat violation of the same on previous dates of 2/3/2026 and 2/17/2026.
