Inspector’s narrative

What the inspector wrote

During the initial inspection dated 2/2/2023 LPA Duron reviewed documents provided by Director informing enrolled families and staff of the positive cases at the facility. The documents states the actions the facility was following to disinfect and sanitize environment. The document states the facility will conduct wellness health checks at arrival and encourage families to monitor any symptoms their child may have, and to inform facility. Director stated if the children or staff tested positive, the children and staff would be quarantined for 5 to 7 days, and return with negative test and symptom free for 24 hours. During the interview with 3 staff, all staff stated Director informs classrooms and families and monitor symptoms, 5 day mask wearing, staff and children who test positive quarantine for 5 days, and return with negative result. Sanitize all the toys, soft toys. Inform families on Kindercare app, email, and in person when they pick up their child. LPA Duron interviewed three parents. All interviewed parents stated they did not have any concern with facility. Based on LPA’s interviews and records review, there is insufficient information to corroborate the allegation of Staff are not following appropriate infection control practice. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is unsubstantiated. Exit interview was conducted. The report was reviewed and discussed. The Notice of Site Visit was posted. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) was provided and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report. All appeals must be in writing and received by the Licensing office within 15 business days. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door.
