What the inspector wrote

Page 1 of 1 A plan of correction inspection was conducted at the facility by Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek. LPA met with Barbara Brown in the office. LPA also met with assistant director, Savannah Garcia in the office. Census was taken in the infant/toddler program. There was a total of 8 infants and toddlers with 2 staff. Third staff was at the door to transition the children. They started transitioning to the toddler room. Six toddlers with two staff transitioned to the toddler room and two infants stayed with one staff in the infant room. Licensee was operating within the licensed capacity as specified on license. The director stated third staff will arrive in the toddler room in 15 minutes. She stated they hired additional staff to come earlier to help out in the infant room. The director and assistant director are available to help out as needed. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 5/28/2024 the facility was in violation of teacher child ratio under section 101415)a)(b) of California Title 22 Regulations. The facility sent a written statement that the issue was taken care of by changing staffs’ scheduling. On today’s inspection LPA learned that the facility has taken care of the issue of teacher child ratio by hiring additional staff. The director Barbara Brown was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. The director, Barbara Brown was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with director, Barbara Brown. End of report
