What the inspector wrote

Page 1 of 1 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite case management inspection of this facility. LPA met with assistant director, Savannah Garcia. The new director, Debbie Rouintree was not present. Two directors from other Kindercare facilities, Barbara Brown and Zelinda Zazuetta arrived during LPA's inspection. Census was taken. There were 4 infants with one staff in Infant Room1, 8 toddlers with 2 staff in Toddler Room. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Today during inspection, LPA found out that the facility has changed director since December 2025 and has not reported this change to the licensing office as it was required. Director's packet has not received in the office for the new director, Debbie Rouintree. In the areas that were evaluated, the following deficiency of reporting requirements - section 101212(b) was observed of the California Code of Regulations, Title 22, Division 12 on today's inspection. Please see next page LIC 809D for detail of deficiency. The facility was provided their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Savannah Garcia. End of report.

    101212(b)- Reporting Requirements- The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s) This requirement was not met as evidenced by: LPA's observation and staffs' interviews today that the change of the center director was not reported to the office. The former director left 12/5/2025. This is a potential risk to the health and safety of children in care.
