What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite case management inspection for the purpose of investigating an incident that the facility self reported to our office on 4/18/2025. LPA met with assistant director, Savannah Garcia. The director, Barbara Brown was not present. Census was taken. There were a total of 3 school age children with one staff observed. According to the self incident report, School age children, Child # 1(C1), Child # 2 (C2), and Child # 3 (C3) reported to Staff # 2 (S2) that S1 had grabbed them. A review of the Facility Personnel Report Summary on this date indicates all facility who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Today LPA interviewed the C1, C2, and C3. C1 did not disclose any such incidents. C2 and C3 stated S1 grabbed their wrists. However, there were discrepancies in C2 and C3's statements. LPA interviewed S1 who denied the allegation. S1 stated that S1 had never touched children physically. S1 remembers one time C1, C2, and C3 were fighting with each other in the classroom, and S1 was trying to separate them by putting their arms in between them to stop the fight. S1 confirmed S1 loves children and keep children busy in different activities to calm children down not to fight. LPA also interviewed 9 other staff who were available at the facility. They did not confirm observing/witnessing such incident from any staff. LPA also interviewed 6 other qualified older children who confirmed they were not hurt by anybody at school. Continued on page 2 Page 2 of 2 Based on the self reported incident, today's interviews with total of 10 staff, and 9 children, there is not a preponderance of evidence/witness to prove the alleged violation did or did not occur. In the areas that were investigated today, no deficiency was issued of the California Code of Regulations, Title 22, Division 12 o n this inspection. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The assistant director, Savannah Garcia was provided a copy of their appeal rights (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted with assistant director, Savannah Garcia. End of report.
