What the inspector wrote

On 12/09/2025, at 12:00pm, Licensing Program Analyst (LPA) Long Pham conducted an unannounced POC inspection. LPA and administrator An Joseph toured the facility. Census was taken in individual classrooms. The overall census observed was 2 staff and 22 preschool children. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:00am. - 6:30pm., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the previous visit on 12/01/2025, LPA observed Staff # 4 Le Emily doesn\342\200\231t have a current criminal record clearance and is taking care of children in Graffe Room. Today, LPA confirmed that Le Emily is not present at the facility, and the deficiency was corrected. The administrator stated that they already sent Le Emily to get the fingerprint on 12/01/2025 and will not allow Le Emily to return to work until fingerprint is cleared. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative An Joseph. Appeal Rights were explained. The Facility Representative was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
