Based on observation, interview, record review, at 1:50pm during facility inspection director provided a copy of Lead testing report, facility conducted their lead test on 04/08/2023, facility was out of compliance due to lead testing was done after 1/1/23. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
