What the inspector wrote

A case management inspection was conducted today by Licensing Program Analyst (LPA) Nunez, who met with Program Lead, Addy Heiber. This inspection was conducted as a follow-up to the Unusual Incident Report (UIR) case management inspection completed on 2/23/26. There were no children present during the visit. Per program lead children start arriving at the program around 12:45 pm today. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Regional Office received a self-reported incident from the facility on 2/17/26. The report stated that Parent #1 (P1) expressed concerns that two staff members caused Child #1 (C1) to feel unsafe at the facility. C1 alleged that one staff member used their fingers in the shape of a gun while playing “robbers” with C1, and another staff member rolled up a blanket and swatted at C1’s bottom. The facility reported that both staff members were placed on administrative leave. During the course of the investigation, LPA interviewed two staff members, five children, and Parent #1 (P1), and reviewed two staff files. Child #1 (C1) no longer attends the program. Page 1 of 2 P1 stated that C1 disclosed not feeling safe at the program. According to P1, C1 reported that Staff #1 (S1) used their fingers in the shape of a gun and pointed them at C1’s head. P1 further stated that C1 disclosed that S1 or Staff#2 (S2) hit C1 on the stomach and bottom. LPA interviewed two staff members and five children. Staff #3 (S3) stated that S1 and S2 were placed on administrative leave after concerns regarding the incident involving C1 were reported. S3 further stated that there had been no prior concerns involving S1 or S2. Staff #4 (S4) stated that S4 had not observed any inappropriate behavior by S1 or S2. All five children interviewed stated that they had not experienced any inappropriate behavior from staff members. LPA was unable to interview S1 and S2 due to their being on administrative leave. LPA was also unable to interview C1 because C1 no longer attends the program. In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
