What the inspector wrote

On 02/23/2026 at 9:00am Licensing Program Analyst (LPA) Long Pham conducted an unannounced case management incident inspection in response to a self-reported Unusual Incident dated 01/26/2026. Upon arrival, LPA met with Sarah Nead-Rendon who led LPA on a tour of the facility. LPA observed 3 staff with 13 preschool children. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 01/26/2026, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 01/16/2026 Staff # 3 (S3) contacted principal regarding few concerns about a preschool instructor that used inappropriate tones with staff, colleagues, and children. On that same day, the principal met with the preschool instructor and began an internal investigation. During today's inspection, LPA conducted a physical plant inspection and interviewed 4 staff, and 2 children. LPA also obtained pertinent documents. Due to the lack of information available currently, the above allegation needs further investigation. Page 1 of 2 Exit interview conducted and report was reviewed with the facility representative Sarah Nead-Rendon. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100. Appeal Rights were explained. The facility representative was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above. End of Report.
