What the inspector wrote

On April 30, 2026, at 9:00am, Licensing Program Analyst (LPA) Long Pham conducted an unannounced case management incident inspection in response to a self-reported Unusual Incident dated 01/26/2026. Upon arrival, LPA met with an Early Learning Program Supervisor Jessica Palacios and was led on a tour of the facility. LPA observed 4 staff with 18 preschool children. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 01/26/2026, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. Staff #5 (S5) reported that on 01/16/2026 Staff #1 (S1) contacted Staff #4 (S4) regarding concerns about Staff #3 (S3) using inappropriate tones with staff, colleagues, and children. On the same day, S4 met with S3 and initiated an internal investigation. S5 received an email from S4 outlining a summary of the meeting. On 01/20/2026, S5 and S4 had a meeting with S3 to discuss the allegations, and S3 was placed on Administrative Leave. During the investigation on 02/23/2026, LPA observed S3 was assigned to complete 4 additional training courses on 01/19/2026; including Children’s Personal Rights in Child Care, Child Care Reporting Requirements, Kenna SafeSchools Training (Communication Styles and Skills), and Review of Board Policy 4119.21(Professional Standards). Page 1 of 2 During the staff interviews conducted on 02/23/2026, S1 stated that on 01/07/2026, S1 observed S3 intentionally stepped on the child #1’s heels (C1). Then S3 asked C1, "Do you like it and how do you feel?" S1 observed C1 replied “OWWWWWW”. On 01/13/2026, S1 observed S3 grab C1’s right upper arm and said, “move it and stay in line” in a very loud voice. S1 also saw S3 yell at C1 when S3 pulled C1 out of the restroom. Staff #2 (S2) stated that on 01/07/2026, S2 saw S3 yell loudly at the child #2 (C2) because C2 was acting out in the classroom. Following S4’s investigation into the three incidents, S4 stated that S3 admitted that S3 intentionally stepped on C1’s heels to correct C1’s behaviors. S5 and S4 met with S3 on 02/19/2026 to discuss the allegations, and they decided to discipline S3 for unprofessional conduct toward children; S3 will remain on Administrative Leave for the remainder of the school year. Based on the information gathered from the self-reported unusual incident report, staff interviews, and records review, it was determined that S3 intentionally stepped on C1’s heels and was placed on Administrative Leave for unprofessional conduct toward children. LPA informed an Early Program Supervisor Jessica Palacios that this report dated 04/30/2026 document(s) One Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. LPA informed an Early Program Supervisor Jessica Palacios to provide a copy of this licensing report dated 04/30/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted and report was reviewed with an Early Learning Program Supervisor Jessica Palacios. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were explained. The director was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
