Page 1 On 11/17/2025, Licensing Program Analyst (LPA), Deschampe conducted an on-site inspection for the purpose of an Unannounced Annual Random inspection. LPA arrived at the facility at 9:00 AM and was allowed entry by the facility representative/program lead director, Diana Aboul-Hosn. LPA explained the purpose of the visit and was guided on a tour of the facility. Center Manager, Cristy Pate arrived at the facility around 1:15 PM. During the tour of the facility, LPA took the following census: 3 staff with 7 children. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 8:00 AM - 6:00 PM, Monday through Friday but primarily operates as an after-school program. A review of the Facility Personnel Report Summary on this date, 11/17/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code (HSC) section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Page 2 During the inspection of the indoor activity space, items which could pose a danger to children (detergents and cleaning compounds) were observed to be stored and out of reach of children. Medications are centrally stored and inaccessible to children. First-aid supplies were complete and stored in each classroom in a location inaccessible to children. Supplies are monitored monthly or, as needed. Food preparation areas were clean and sanitary. Food is properly stored. Menus posted where they could be reviewed by parents/authorized representatives. Water is provided by individual water bottles with refills supplied by a water pitcher. Fixtures, furniture, equipment, and supplies were clean and observed to be in good repair, free of sharp edges, and age appropriate for all children enrolled in care. The children's and staff bathrooms are safe and in sanitary operating condition. The toilet facilities provide individual privacy. The facility has conducted an emergency drill within the past six months (11/10/2025). The facility has a centralized smoke detection system tested on 08/08/2025 and a working carbon monoxide detector (tested and operable). There is 1 fire extinguisher on site, was serviced on 10/17/2025. The facility maintains an indoor temperature within regulation (minimum of 68 degrees and maximum of 85 degrees F or, in extreme heat a maximum of 20 degrees less than the outside temperature. Facility met all waiver, exceptions, and posting requirements , including the California Child Passenger Safety Law, posted by the entrance of the facility. The outdoor activity spaces were inspected for compliance. The play area was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. Drinking water in the outdoor activity space is provided by personalized water bottles and refilled with a water pitcher. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the facility representative shall test their water for lead within 180 Page 3 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Staff files were reviewed for staff present during the facility inspection on this date, 1 out of 1 staff files were reviewed. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (HSC) 1597.622 states, a person shall not be employed or volunteer at a child care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis and measles for staff were reviewed and within compliance. Beginning March 31, 2018, HSC 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications. A random sample of 5 children's records were reviewed. There was a separate, complete and current record for each child. In the areas reviewed 5 out of 5 children\342\200\231s files were found to be in full compliance. Sign in/out is completed by the parents/authorized representatives using Hubbe with date/time, complete signature, and personal PIN. LPA also informed facility representative of the importance of checking for recalled devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all devices with the CPSC to be notified of any recalls on their purchased equipment. Page 4 The facility representative was informed that Licensing Quarterly Updates are available at www.ccld.ca.gov and may request to be added to an email list to receive a Quarterly Update from www.ccld.ca.gov and select Receive Important Updates link. LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Exit interview was conducted and report was reviewed with the facility representative facility representative/center manager, Christy Pate. Appeal Rights were discussed with facility representative. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on, or immediately adjacent to, the interior side of the main door to the facility.
