Licensing Program Analysts (LPA) Silva conducted a unannounced Case Management \342\200\223 Incident to investigate a report dated 5/2/2025. Upon arrival, the LPA met with Director Ruby Porter. The facility personnel summary shows all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The census was 19 children supervised by qualified staff in room 1, 2, and 3. During the visit, the LPA interviewed seven staff and one parent about the self reported incident. The LPA obtained documents relevant to the investigation. There were no violations at the time of the visit. Additional information is needed to make a final determination. This licensing report was reviewed with the facility representative. A copy of the appeal rights was provided, and the appeal rights discussed. A notice of site visit was provided and posted. The notice of site visit must be posted for 30 consecutive days.
