What the inspector wrote

Page 2 of 2 On the inspection day of 6/4/2024, LPA observed 2 staff and one volunteer with 14 preschool children on the playground, one staff with 9 preschool children in Blue Room, and another staff with 9 preschool children in Green Room. Additional volunteer was setting the nap equipment up for children’s napping in the classroom. On that day, LPA interviewed 4 staff and two volunteers who did not state that they were out of ratio. LPA interviewed six children who were not able to determine the number of staff to number of children daily. During the course of investigation, LPA also attempted to contact 3 former staff. However, LPA was successful to interview one former staff who denied facility being out of ratio. LPA contacted 10 parents of whom one responded with positive feedback. LPA reviewed the timecards of staff randomly for the dates of Thursday May 30, Friday May 31, Monday June 3rd, and Tuesday June 4th. The hours of staff # 2 were unknown due to no timecard being available for staff # 2. Staff # 2 stated staff was working at the facility. LPA reviewed the sign in out sheet for students for those dates. Based on interviewing 5 staff, two volunteers, ten parents of whom one responded, and 6 children, and LPA’s observation of 6/4/2024 and today, there is not enough proof or evidence to support the above allegation occurred or did not occur. This agency has investigated the complaint alleging “facility is out of ratio” although the allegation may have happened or is valid, there is not a preponderance of evidence to prove, the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. In the areas which were investigated, no deficiency was cited today. The director, Stephanie Vera was provided a copy of their appeal rights (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted with director, Stephanie Vera. This report ends here.
