What the inspector wrote

An unannounced case management inspection was conducted on 09/15/2025 by Licensing Program Analyst (LPA), Deschampe arrived at the facility at 2:05 PM, in response to a self-reported Unusual Incident Report (UIR) dated 09/05/2025. Present during today’s inspection was the Facility Representative/Director, Hope Flores who left at approximately 2:30 PM. Facility Representative/Support Teacher, Ellen Long guided LPA on a tour of the facility. Census was taken as follows: 39 napping children and 6 staff. A review of staff criminal records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 09/05/2025, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident that allegedly occurred on 09/03/2025. Adult 1 (A1) stated on the report that Child 1 (C1) alleged Child 2 (C2) inappropriately touched C1 in the classroom. LPA received a copy of the UIR on 09/05/2025. Per the UIR, Adult 3 (A3) reported C1's sibling reported the alleged inappropriate touching to C1's Authorized Representative (AR). The AR then reported the alleged incident to the facility. The facility conducted an interview with C1 and A3. As a result of the facility's investigation into the matter, the facility decided to disenroll C2 as of 09/04/2025. On this date, LPA made observations, received facility records, attempted to interview C1, and conducted interviews with 2 staff. Due to additional information needed, the above allegation needs further investigation. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the OCRO within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative, Ellen Long. End of report.
