What the inspector wrote

On 9/17/2025, at 12:15pm, Licensing Program Analyst (LPA) Anna Chan conducted a case management visit. Upon arrival, LPA met with Director Taryn Stephens. LPA was led on a walk through of the facility and census was taken. There were 20 infants and 3 staff (Room #1- 7 infants with 2 staff, Room #2- 13 infants with 1 staff) Children were napping when LPA arrived. During walk through at the facility, LPA observed 13 napping infants with 1 staff present in what they call “Toddler Room” or Room #2. The director stated infants in Room#2 "Toddler Room" were ages 12 to 24 months. LPA reminded the facility that napping infants’ ratio is up to 12 napping infants with 1 staff. Licensee Deborah Cosby stated, they will move one infant to the other infant room to be in ratio. LPA interviewed staff 1 (S1), S1 stated they were aware that there were 13 infants napping during walk through and confirmed that 1 child was transferred to the other infant room to keep ratio. Based on observation and interview, one (1) Type B violation was cited under the California Code of Regulations Title 22 Div 12, Section 101416.5(d) Staff-Infant Ratio. Director was provided with a copy of California Code of Regulation Title 22 Div 12, Section 101416.5(d) Staff-Infant Ratio. Exit interview was conducted and report was reviewed with director Taryn Stephens. The Notice of Site Visit was posted and must be posted for 30 consecutive days. Appeal Rights were provided
