What the inspector wrote

On 05/19/2026, Licensing Program Analyst (LPA) K. Trinh conducted a Plan of Correction (POC) inspection in response to deficiencies cited on 4/22/2026. LPA met with licensee, Mirna Palmeri. During the inspection there were two (2) infants and five (5) preschool age children present. A review of the Facility Personnel Report Summary on this date indicates all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today's visit, LPA did not observe infants napping in a play yard or crib with loose objects and articles; infants napping with a pacifier with any attachments; or infants in strollers when it is not used for transportation. The three (3) deficiencies that were previously cited on a LIC 809 dated 04/22/2026 have been cleared. Appeal Rights were explained. The licensee was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee, Mirna Palmeri.
