Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide a file for staff #2 (S2) during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

OPERATION OF A FAMILY CHILD CARE HOME
Based on observation and interview, the licensee did not comply with the section cited above. LPA observed an uncharged 2A-10BC fire extinguisher inside the kitchen. Licensee was reminded The pressure gauge on the 2A-10BC fire extinguisher(s) need to indicate fully charged which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee stated there are no other fire extinguishers inside the home.

OPERATION OF A FAMILY CHILD CARE HOME
Based on observation, the licensee did not comply with the section cited above. While touring the home at 12:24 p.m. LPA observed facial products in the designated childrens restroom inside an unlocked cabinet. On the back of the product it reads "keep out of reach of children which poses an immediate health, safety or personal rights risk to persons in care.

Licensee was unable to provide documentation showing a fire/disaster drill is being conducted at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 3 out of 11 children were missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.
