Based on observation, the licensee did not comply with the section cited above. While touring the designated childrens bathroom at 1:49 PM, LPA observed peroxide inside an unlocked cabinet and shaving razors inside an unlocked drawer which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. While touring the back yard of the family child care home, LPA observed an open side gate leading to an open shed. Inside the shed, LPA observed Ortho bug spray, Grout Haze Remover, and tree pruners with saw blades which were accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

While touring the inside of the home at 1:46 PM, LPA observed cribs with blankets and sheets hanging above and attached to the side of the cribs which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and Assistant did not have current mandated reporter certifications. Mandated reporter certifications for licensee and assistant expired 8/2/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidence by reviewing the files at 2:30 pm. The licensee, and her assistant did not have the mandated reporter training on file. They were expired 11/10/20. This poses a potential risk to the health and safety of children in care.
