Page One On 11/19/2024, Licensing Program Analyst (LPA), Alma Castro, conducted an onsite inspection for the purpose of an Annual Random. LPA and Mina Assadzadeh toured the facility inside and outside and the floor and yard plan were verified. Census was taken: 2 infants and 5 preschool children. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:30am to 5:00pm., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; lunch and snacks are provided. Food prep areas were clean and sanitary. Food is properly stored. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by sports bottle. The children's bathroom is clean and sanitary. Children nap on cots/toddler beds, and bedding is stored individually and facility washes blankets ever other day. The facility has conducted an emergency drill within the past six months. The last drill was conducted on 11/08/2024. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement s. The California Child Passenger Safety Law was posted by the entrance of the facility Page 1 of 4 Page 2 The home has age-appropriate toys for the ages served. LPAs verified there is a working telephone service (cellular service). Licensee stated, they use the back yard for outdoor play, LPAs inspected the back yard, and it was found to be in compliance. There are no bodies of water at the facility. The licensee Pediatric CPR/First Aid certifications were up to date. 3 out of 3 staff certifications were up to date. Beginning September 1, 2016, Health, and Safety (H&S) 1597.622 (a)(1) states a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles . Proof of immunization against pertussis, measles, was reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years. Upon file review, it was found that 3 out of 3 staff had the Mandated Reporter Training certificate on file. The licensee understands they must be present in the facility and must ensure children in care are always supervised. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training. CCLD website www.ccld.ca.gov was provided to licensee to access regulations, updates, and licensing forms. Licensee was advised to register through childcareadvocatesprogram@dss.ca.gov in order to receive quarterly updates. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. Page 2 of 4 Page 3 The licensee has a current roster of children in care. 7 children\342\200\231s records for children present during LPA\342\200\231s inspection were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700), Immunization records, Consent for Emergency Medical Treatment (LIC627), Notification of Parent\342\200\231s Rights (LIC995A) and found to be in compliance. 1 of 7 children\342\200\231s records reviewed was missing immunization record/exemption to immunization documents. The licensee Pediatric CPR/First Aid certification expires 03/2026. 3 out of 3 staff files were reviewed. Facility does not provide IMS. For information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee, Mina Assadzadeh was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care- licensing/subscribe and select the Child Care option to receive email communication. (The licensee is already signed up for this purpose). Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. In the areas that were evaluated, the following deficiency was observed of the California Code of Regulations, Title 22, Division 12 on this inspection. Please see LIC 809D for details of deficiency. Exit interview conducted and report was reviewed with the licensee Mina Assadzadeh. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above.
