What the inspector wrote

Licensing Program Analyst (LPA) Aiddee Nunez conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 9/9/2025. During today’s visit, LPA met with licensee Angelica Barajas. LPA observed at the home the Licensee, one adult assistant and 5 children which included: 3 preschool age children and 2 infant age children. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Today's Case Management inspection is being conducted in response to a self-reported incident, reported to the OC CCL office on 9/9/25, detailing on 9/8/2025, Parent#1 (P1) claimed to another day care parent that Adult#1 (A1) had touched Child#1 (C1) inappropriately in the daycare. During today's visit, LPA interviewed the licensee, 2 staff members, A1, and 3 children. LPA also obtained a copy of the children’s roster. Additional information is needed to complete this investigation. Page 1 of 2 No deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Exit interview conducted and report was reviewed with Licensee, Angelica Barajas. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights discussed. Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. Page 2 of 2
