What the inspector wrote

On 5/29/2026, Licensing Program Analyst (LPA) Madeline Do arrived at the Family Child Care Home (FCCH) for the purpose of an Annual Inspection and met with licensee PhuongDung Duong and was led on a tour of the inside and outside areas of the home. There were zero children present. Licensee stated hours of operation are 8 am – 6 pm Monday through Friday. Off limits areas are made inaccessible by means of locked doors. The daycare area consists of a living room and one bathroom. There was a working carbon monoxide, smoke detector, and a fire extinguisher in the home that met statutory requirements. Detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. Licensee stated there were no poisons or other items which could pose a danger to children. If they were observed, they were locked or inaccessible. Licensee Duong stated there were no firearms and/or weapons in the home. The home had age-appropriate toys. LPA Do verified there was a working cellular telephone service. Licensee was reminded that childcare phone must always remain in the home. Licensee Name stated that they use the front patio for outdoor play area. Licensee stated the children bring meals and snacks from home. Licensee stated they conducted an emergency drill within the past six months in December 2025. However, licensee did not keep documentation of the drills. Licensee created documentation during this inspection. Licensee stated they understood that they must be present in the home and must ensure children in care are always supervised. When Licensee is temporarily absent from the home, arrangements must be made for a qualified substitute adult to care and supervise children while absent. Continue to Page 2 Page 2 The substitute adult must have the required criminal record, child abuse index clearances, immunization, Pediatric CPR/First Aid, and mandated reporter training. LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian The Facility Personnel Report Summary reviewed indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Currently, there are two (2) adults including the licensee living in the home. Children's records were reviewed, and there was a separate, complete, and current record for 3 out of 3 children. Licensee registered for mandated reporting training in Vietnamese during this inspection. Licensee possesses expired EMSA approved Pediatric CPR/First Aid certification, which expired 4/21/2026. (See LIC809D for deficiency cited.) Licensee stated that she had immunization exemption but did not have it for LPA to review. Licensee contacted the doctor’s office to obtain the exemption during this inspection. CCLD website www.cdss.ca.gov/inforesources/community-care-licensing was provided to Licensee to access regulations, updates, and licensing forms. Licensee was advised to register through childcareadvocatesprogram@dss.ca.gov in order to receive quarterly updates. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. In the areas that were evaluated, 1 Type B deficiency was observed in the California Code of Regulations, Title 22, Division 12 at the time of the visit. Licensee Duong confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO Profile Check on 05/29/2026.
