Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Based on observation, interview, and record review, the licensee did not comply with the section cited above in that, 2 out of 3 staff did not have influenza verification on file, which poses a potential health, safety or personal rights risk to persons in care.
