    Based on interview and record review, the licensee did not comply with the section cited above in that, licensee and assistants did not have proof of immunization against influenza (or written declaration to decline) and Assistant 2 (A2) was missing record of measles and pertussis, which poses/posed a potential health, safety or personal rights risk to persons in care.

  • STAFFING RATIO AND CAPACITY

    Based on observation and interview, the licensee did not comply with the section cited above. LPA entered the home at 1:53 pm and observed assistant Nellily Serrano caring for nine children inside the family room area of the home. There were no other adults present until licensee arrived at 2:21 pm.in [count] out of [total count] [(objects) (persons)] [identifiers] which poses an immediate health, safety or personal rights risk to persons in care.

    PERSONNEL REQUIREMENTS

    Based on observation and interview, the licensee did not comply with the section cited above. Assistant present could not provide proof that shows she has a current Pediatric cardiopulmonary resuscitation and pediatric first aid certification which poses an immediate health, safety or personal rights risk to persons in care.

  • OPERATION OF A FAMILY CHILD CARE HOME

    Based on observation and interview, the licensee did not comply with the section cited above. While touring the inside of the home at 1:58 pm, LPA observed an unlocked kitchen drawer with knives inside. Children in care has access to the kitchen area which makes this an immediate safety risk to children in care. Licensee stated the lock has been broke for months but havent fixed it which poses an immediate health, safety or personal rights risk to persons in care.

  • OPERATION OF A FAMILY CHILD CARE HOME

    Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee provided LPA with a fire/disaster drill log and last drill was conducted on 10/21/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee mandated reporter expired 8/17/2021. Licensee and Assistant was unable to provide copies of their mandated reporter certification. Licensee stated she was unaware of the training. LPA observed licensee was cited for the mandated reporter on 7/10/2019 which poses/posed a potential health, safety or personal rights risk to persons in care.
