What Happened
Type A and Type B violations identified. Licensee absent at time of arrival (arrived ~10:40am); two assistants caring for 12 children without current CPR/First Aid certifications or mandated reporter training. Fire/disaster drills overdue (last conducted 10/14/2022, should be every 6 months).
Observations
- Licensee Yifan Yao not present at time of arrival - two assistants S1 and S2 caring for 12 children
- Assistants S1 and S2 lack current Pediatric CPR/First Aid certification
- Assistants S1 and S2 unable to provide proof of mandated reporter training
- Licensee's mandated reporter certification expired 08/19/2021
- Last fire/disaster drill conducted 10/14/2022 - overdue (should be every 6 months)

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee mandated reporter certification expired 8/19/2021. Assistants present were unable to provide proof of their mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
