What the inspector wrote

On 12/11/2025, a random Annual inspection was conducted at the facility by Licensing Program Analyst (LPA) Dianna Valdez Santana and met with licensee, Fanny Soria Chavez and census was taken. LPA observed the licensee and assistant caring for 4 children (2 infants, 2 preschool-age children). Licensee was operating within the licensed capacity as specified on license. A review of the Facility Personnel Report Summary on today\342\200\231s date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently there are four adults, including the licensee living in the facility. Facility Day care hours are 3:30 am-6:00pm, Monday through Friday, Saturdays 4:30am-2pm. During today\342\200\231s inspection, LPA and licensee toured the inside and outside areas identified in the facility sketch as accessible to childcare children. Off-limit areas are made inaccessible by means of child safety gates and locked doors. The facility is a one-story home with 4 bedrooms and 2 bathrooms and an outside children\342\200\231s bathroom. The childcare area consists of the family room (main daycare area), living room, kitchen, two bathrooms and the backyard. The licensees stated the main day-care area is the family room. There are working carbon monoxide, smoke detector, and fire extinguisher in the home that meet statutory and State Fire Marshall standards. detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. Licensee stated there are no firearms and/or other dangerous weapons in the facility, and none were observed during today's inspection. There is a fireplace in the living room screened by a metal cover and inaccessible to children in care. Page 1 of 4 Page 2 The home has age-appropriate toys for the ages served. LPA verified there is a working telephone service, which is the licensee\342\200\231s cell phone. Licensee stated they use the back yard for the children\342\200\231s outdoor play area. There were no poisons or other items observed which could pose a danger to children. There are no bodies of water present at the facility, The licensee had a current roster of children in care. Five random Children\342\200\231s records were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700) and were found to be in compliance. Licensee stated the facility has conducted emergency drills within the past six months. Last drill conducted on 11/10/2025. LPA reviewed 15-minute Sleep Log and LIC9227 for two infants present and was found to be within compliance. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for licensee and assistant were reviewed and within compliance. The licensee\342\200\231s American Heart Association, Pediatric CPR/First Aid certification expires 6/2026. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years. The licensee\342\200\231s Mandated Reporter Training Certificate expires 2/5/2027. CCLD website www.ccld.ca.gov was provided to licensee to access regulations, updates, and licensing forms. Licensee was advised to register through childcareadvocatesprogram@dss.ca.gov in order to receive quarterly updates. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. Page 2 of 4 Page 3 LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. During the exit interview, the Licensee, Fanny Soria Chavez confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Appeal Rights were given and the notice of site visit was given and must remain posted for 30 days. Report was translated in Spanish for the licensee by the LPA. Page 4 of 4. End of Report.
