On 10/16/2025 Licensing Program Analyst (LPA), K. Trinh conducted an onsite inspection for the purpose of an Annual Random. LPA observed Staff 2 (S2) caring for 12 children which included 1 infant and 11 preschool age children. At approximately 1:53PM, LPA met with the Licensee. Licensee was operating within the licensed capacity as specified on license. Facility Day care hours are 8:00AM-6:00PM, Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. During today\342\200\231s inspection, LPA and licensee toured the inside and outside areas identified in the facility sketch as accessible to child care children. Off limits areas are made inaccessible by means of a baby gate. The child care area consists of the bedroom, living room, family room, dining area, kitchen, 2 children's bathroom, and backyard. There are working carbon monoxide, smoke detector, and fire extinguishers in the home that meet statutory and State Fire Marshall standards. (Continue to page 2) (Page 2) Detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. There were no poisons or other items observed which could pose a danger to children or if they were observed, they were locked or inaccessible. Licensee stated there are no firearms and/or other dangerous weapons in the facility and none were observed during today's inspections. There was a fireplace in the living room and was inaccessible to children in care. The home has age-appropriate toys for the ages served. LPA verified there is a working telephone service (cellular service), licensee was reminded that childcare phone needs to remain in the childcare at all times. There were no bodies of water on the premises. During today\342\200\231s inspection, LPA observed an infant sleeping in a recalled rocker; see LIC 809D for deficiency. LPA informed the licensee about checking for recalled devices on the United States Consumer Product Safety Commission (CPSC) website. LPA also texted licensee the link to the CPSC recall specifically on the device observed at the facility. Based on LPA\342\200\231s record review and interview, the facility logs children\342\200\231s start and end sleep time on the ProCare app. The app send notification reminders for caregivers to check on sleeping children. Licensee and Staff 3 (S3) stated that they physically check on the children when the app notifies them every 15 minutes. Facility does not have a record on the app or physical documentation their safe sleep visual checks; see LIC 809D for deficiency. Licensee\342\200\231s file was reviewed during the facility inspection on this date. At least one assistant possesses a current EMSA approved Pediatric CPR/First Aid certifications, which expires 01/2026. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family childcare home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against influenza, pertussis, and measles for licensee was reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training and to renew the training every two years. (Continue to page 3) (Page 3) Four children's records were reviewed, and there was a separate, complete, and current record for four children\342\200\231s files. In the areas reviewed, the children\342\200\231s files were found to be in full compliance. The outdoor activity space was inspected for compliance. The space was enclosed by a fence at least five feet in height. The surface of the outdoor activity space was maintained and free of hazards. There were age-appropriate toys and materials for children in care. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care for and supervise children while absent. The substitute adult must have the required criminal record clearance, child abuse index clearance, immunizations, Pediatric CPR/First Aid, and mandated reporter training. Licensee was advised to register through childcareadvocatesprogram@dss.ca.gov in order to receive quarterly updates. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. (Continue to page 4) (Page 4) LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The facility was not in compliance and violations of the California Code of Regulations, Title 22, Division 12, were observed, discussed and cited at the time of the visit. The following violations of the California Code of Regulations, Title 22; Division 12, were observed and cited today: 1 Type A and 1 Type B (see LIC 809D for deficiencies). Due to the Type A violation cited today, the licensee shall post, and provide copies, of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. The licensee is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child\342\200\231s file. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. (Continue to page 5) (Page 5) During the exit interview, licensee, Tatiana Miranovich, confirmed that there are no Registered Sex Offenders living in the facility, and LPA completed the RSO profile in FAS. Appeal Rights were explained. The Licensee was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above.
