Page 1 of 1 A plan of correction inspection was conducted at the facility by Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek. At 8:30 am LPA met with licensee's assistant, Natalia Martinez Xocua who was supervising 5 children of whom 3 were infants and 2 were over 2 years of age. The licensee, Melissa Linan arrived approximately at 8:45am. Another child under 24 months old was added to the census. Therefore the total of children are 6 children of whom 4 are under 24 months old with licensee and her assistant. Licensee was operating within the licensed capacity as specified on license. Day care hours of operation is Monday through Friday from 7:00 am-5:30 pm. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 9/5/2024 the facility was in violation of staffing ratio and capacity under section 102416.5(a) of California Title 22 Regulations by having 4 children under 24 months old and 2 preschool children without having any assistant. A citation was issued on 9/5/2024. The facility sent a plan of correction to LPA stating she is aware of the staffing ratio and she would comply with it at all times. On today\342\200\231s inspection, LPA learned that the facility has taken care of the issue of staffing ratio by having her assistant present when is large capacity. The licensee, Melissa Linan was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. The licensee, Melissa Linan was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with licensee, Melissa Linan. End of report
