What the inspector wrote

Licensing Program Analyst (LPA) R. Castanon conducted an unannounced case management visit. Upon arrival, LPA met with Licensee Emilia Montano. Observed at the time of the visit was a total of 6 children and 1 assistant. A review of the Facility Personnel Report Summary on 12/08/2023 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA explained the reason for today\342\200\231s visit to Licensee Emilia Montano. The licensing department's report system does not reflect forms provided to Licensee during previous visits conducted on 09/15/2023 and 10/09/2023. LPA provided Licensee with copies of LIC9058 Appeal Rights on this day for those visits. Exit interview was conducted with Licensee Emilia Montano.
