What the inspector wrote

On December 30, 2025, at 1:00pm, Licensing Program Analysts (LPAs) Long Pham and Dean Thompson conducted an inspection for the purpose of investigating the complaint's allegation. LPAs met with Licensee Holland Jacqueline who led LPAs on a tour of the facility. During today’s visit, there were no children in care. A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today's inspection, LPAs Pham and Thompson conducted a physical plant inspection and interviewed licensee. LPAs also obtained pertinent documents. On 12/30/2025, the interview stopped at 11:57am because the Licensee requested to close her facility and surrendered her License at 11:57am. Licensee stated that I am no longer interested in being licensed and will be operating as a parent co-op. LPA also explained that providing unlicensed care was a violation of the CA Health & Safety Code, Sec. 1596.80. Suite # 250 Orange, CA 92868. Phone # 714-703-280 Page 1 of 2 Exit interview conducted and report was reviewed with the Licensee Holland Jacqueline. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100. Appeal Rights were explained. The facility representative was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above. End of Report.
