What the inspector wrote

An Informal Meeting was conducted on Wednesday, May 27, 2026, at 10:00am, in the Orange County Regional Office (OCRO). Present during the meeting were Licensing Program Manager (LPM) Tina Nguyen, Licensing Program Analyst (LPA) Long Pham, and Licensee Holland Jacqueline. The purpose of the informal meeting was to review the history of the licensed day care and recent inspections. The goal of this conference is to support the Licensee in achieving and maintaining compliance with the Title 22 California Code of Regulations. During the meeting, LPM Nguyen discussed the following unlicensed complaints: · 01/06/2026 – 2199 National Ave., Costa Mesa, CA 92627 – Facility #304315099 · 02/07/2024 – 970 Governor St., Costa Mesa, CA 92627 – Facility #304315053 This informal meeting also reviewed the following regulations: · 102357 Operation Without a License · 102393 Unlicensed Facility Penalties · 102358 License Exemptions Page 1 of 2 The Licensee was provided with an opportunity to discuss the reasons for the deficiencies and to present a detailed plan for achieving compliance with California Title 22 Health and Safety Codes (HSC) and the California Code of Regulations (CCR). Licensee was reminded that: • The Department’s mission is to protect the health and safety of the children in the community care facilities; it is essential to be transparent in communication and maintain professionalism during LPA’s visits at the facility. • The licensee will be held accountable to be in compliance with Title 22 HSC & CCR. Should Licensee have any questions, Licensee can consult with Officer on Duty by contacting the OCRO main line at 714-703-2800. • The licensee was advised to check the Childcare Licensing web site at www.ccld.ca.gov for quarterly updates, training, forms, and regulations. • Technical Support Program (TSP) information was offered to Licensee. Licensee denied to receive the service at this time. Exit interview was conducted and report was reviewed with Licensee Holland Jacqueline. Appeal Rights were explained. The Facility Representative was provided with a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of this office visit report and these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above. End of Report.
