What the inspector wrote
On 2/12/2025 at 9:05am, Licensing Program Analyst (LPA) Long Pham conducted a case management inspection, in response to licensee\342\200\231s request for a capacity change from a small to a large Family Child Care Home. LPA met with licensee Helen Lee, who provided a tour of the home. LPA observed licensee Helen Lee and an assistant Chae Shin providing care for 5 preschool children. The Licensee was operating within the licensed capacity as specified on license.
Licensee stated she is not currently registered with any Foster Care agency or holds a foster parent license. Licensee was reminded if changes are made to notify the licensing office.
A review of the Facility Personnel Report Summary on this date indicates all residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee stated that there are 2 adults and 1 minor including the licensee residing in the facility currently. Facility Day care hours are 8:00am \342\200\223 4:30pm, Monday through Friday.
All areas on the Facility Sketch LIC 999 were inspected, including but not limited to, off-limits areas. This is a two- story home with 4 bedrooms, 3 bathrooms, the living room, the kitchen, the front patio (fenced), the backyard (fenced), and a garage. The facility doesn\342\200\231t have a fireplace.

Based on the Facility Sketch LIC999 submitted, areas off-limits to children and parents are as follows:
Licensee has designated 3 bedrooms, 2 bathrooms, the kitchen, the front patio (fenced), and the garage as the off-limits areas. The licensee has a child-proof safety gate in front of the 2
nd
floor\342\200\231s stair, ensuring the 2
nd
floor is inaccessible to children in care. The licensee has a child-proof safety lock on the door of the garage ensuring this area is inaccessible to the children in care.
The licensee understands that licensing staff may have access to off-limits areas during inspection visits if necessary.
Areas Designated for Day Care Activities:
The licensee has designated the living room, the bedroom number 1, the bathroom number 1, and the backyard (fenced) as the day care areas. The children will nap in the bedroom number 1.
Garage will not be used for day-care. The licensee stated the garage will be off-limits. Day-care children are not to nap or eat in the Garage.
Detergents, cleaning compounds, medicines, sharp objects, and hazardous items that can pose a danger to children are inaccessible in areas designated for children. The licensee stated that there are no poisons on the premises and LPA did not observe any. The licensee was advised that any poisons must be locked with a key or combination lock. The licensee stated that there are no weapons or firearms on the premises. The LPA informed the licensee that when firearms are present, they must be locked and stored separately from the ammunition per CCR 102417(g)(4)(C).
The pressure gauge on the 2A-10BC fire extinguisher(s) indicates fully charged. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors were tested and are operable. Smoke and carbon monoxide detectors should be checked regularly and batteries replaced as needed. The pull fire alarm required by the fire department is located in the main entrance hallway.
Outdoor play activities will be conducted in the backyard (fenced). The backyard is appropriately fenced. LPA observed age-appropriate play equipment to be free from hazards. There are no bodies of water on the premises.

A records review indicates the licensee completed the required Health and Safety Training with Nutrition and Lead Poisoning components. The licensee has a current Pediatric First Aid and CPR certification that expires on 06/26/2027.
In the absence of the licensee, a qualified adult must be present to supervise the children. A qualified adult is an individual who has a valid and current Pediatric First Aid and CPR certification, TB clearance and immunization, and valid criminal record clearance that is associated with the licensed facility.
Annual fees must be paid by the due date, or a late fee shall be assessed and/or the License may be terminated. The licensee was reminded that all licensees must be present at least 80 percent of the hours of operation per day.
H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee has submitted proof of immunizations.
Health and Safety Code 1596.7995: Beginning January 1, 2018, all licensed providers, applicants, directors, and employees must complete training as specified on mandated reporter duties. Training is available at

. The licensee has completed the required mandated reporter training. Renewal is required every two years. Licensee\342\200\231s mandated reporter certification expires on 11/06/2026.
Safe Sleep - Family Child Care Homes. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at:

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended them to register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the
MyChildCarePlan.org website
; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Family Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
During the exit interview, the licensee Helen Lee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
A new license for operating a Large Family Child Care Home shall be issued upon final review. In the event additional requirements are needed, the licensee will be notified. A license will be issued once all requirements are met.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the licensee Helen Lee.
Appeal Rights were explained. The Licensee was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above.
